Forensic Consultation Fee Schedule

Detailed below is our current fee schedule.  We have invested careful thought and consideration in preparing this based on our operation experience.  Our foremost priority is to continue to provide timely high quality professional services.

Document Packet - The purpose letter, complete legal/medical records should be received in our office at least 10 calendar days prior to the date of service.  your presenting legible, well-organized records will facilitate the preparation of a high quality report.  If disorganized records are received, there will be additional charges for our office to organize the records.

• Late receipt of document packet            $ 50.00 per day (up to a $500.00 maximum)
• Records Organization                            $50.00 per hour

Independent Mental Health Evaluation - Fee includes patient history and evaluations, a typed report, up to two (2) hours of document review of the materials delivered to us and up to a quarter (0.25) hour of phone conference time.

• Prepayment                           $1,000.00 (due one (1) calendar week in advance)
• Post payment                         $1,100.00 
• Late payment                         $1,200.00 (30 days or more after appointment)
• Cancellation                           $500.00 (less than three (3) working days in advance)
• No-show fee                           $500.00
• Document Review                  $250.00 per hour (in excess of two (2) hours)
• Supplemental Reports            $250.00 per hour
• Administrative Time             $50.00 per hour (for follow-up to obtain documents)

Reports - Every effort will be made to provide a completed typed report within (5) working days after the evaluation is complete.  If you will need a completed report more quickly than that, please notify us at the time the appointment is scheduled.

• Accelerated report generation       $200.00 per day
• Next-day report generation           $1000.00

Document Review - There is a minimum of one (1) hour minimum charge.

• Review                          $250.00 per hour
• Written Report              $250.00
• Supplemental Reports    $250.00 per hour
• Administrative Time      $50.00 per hour (for follow-up to obtain documents)

Depositions and Arbitration Hearings - There is a one (1) hour minimum charge, which is prepaid as a non-refundable retainer applicable to date(s) rescheduled by the arbitrator/arbitration panel.  Services provided outside of our geographical area (50 mile radius from office) are charged on a per diem basis.

• Deposition                            $500.00 per hour (four (4) hour minimum)
• Arbitration hearing              $500.00 per hour
• Out of Geographic Area       $3,000 per diem

Trial Testimony - There is a four (4) hour minimum prepaid, non-refundable retainer applicable at the time testimony is scheduled.  This retainer is applicable to date(s) rescheduled by the court.  Services provide outside our geographical area (50 mile radius from office) are charged the above listed per diem rate.  Travel is included in the four (4) hour minimum.

• Trial Testimony                   $500.00 per hour

A minimum retainer in the amount of $2000.00 will be required prior to beginning work on new case assignments for un-established accounts, and will be held and applied to the last invoice.  Prepayment of a retainer will be required for deposition, courtroom, or other legal testimony/appearance, based upon the estimated amount of time require to perform the function and the geographic-specific rates quoted above, at least seven (7) working days prior to the scheduled event.
